Bites not killing you CAN be compelling gameplay if you make it so that recovering from the illness is difficult. As it stands, a bite being a guaranteed death sentence (or any injury after getting the symptoms) means that you are a dead man walking, and after a certain amount of attempts it loses its ludonarrative lustre.
