Basically you will miss out on Zote's full storyline, but get an achieve in return. There's also a 2nd chance to leave him to die, when he's caught up in spiderwebs in the deep nest. If you rescue him both times, then you'll miss the 'leave to die' achieve but be able to see his whole story arc. #6.
